Passport Application Requirements

All passport applicants are requested to carefully read the requirements for their application and make sure that they bring all necessary requirements on the date and time of their scheduled appointment to avoid inconvenience.

A. FOR Passport Renewal

    1. Schedule passport appointment via the Book Appointment Link.
    2. Personal Appearance
    3. Print-Out of confirmed online passport application form.
    4. Original passport and one (1) copy of passport data page
    5. Processing Fee of 8,400 Japanese Yen (cash only)
    6. Self-addressed envelope (nagagata 3) with 660 yen stamps (Japan Post yubin kite). On said envelope, the name of the applicant MUST BE WRITTEN IN ROMAJI, and do not forget to write your contact number.
    7. For change to husband’s surname
      • Original and one copy of Report of Marriage issued by the Embassy/Consulate (if less than 1 year from date of marriage in Japan); or original copy of Philippine Statistics Authority (PSA) marriage certificate
    8. For reversion to maiden name
      • Original PSA birth certificate; and
      • If divorced, original PSA marriage certificate with annotation of divorce, certified true copy (CTC) of the Divorce Decree, and DFA-authenticated certified true copy of Philippine judicial recognition of foreign divorce decree with certificate of finality.
      • Original PSA Advisory on Marriage.
      • If widowed, original PSA death certificate of the deceased husband, or certified copy of Shibo Todoke and Kosekitohon.
      • If marriage is annulled, original PSA marriage certificate with annotation of annulment, and DFA-authenticated certified true copy of court order with certificate of finality.
      • Original PSA Advisory on Marriage.
    9. For Minors (less than 18 years old)
      • Print-Out of confirmed online passport application form.
      • Copy of Embassy/Consulate Report of Birth, if born in Japan, and/or copy of PSA birth certificate, if not born in Japan;
      • Copy of parent's PSA marriage certificate to establish parental authority.
      • Copy of passport of the accompanying mother. If accompanied by the father, copy of parent's PSA marriage certificate to establish parental authority.

B. FOR newborn and first-time passport applicants

      1. Schedule passport appointment via the Book Appointment Link or apply for passport same time as Report of Birth (additional expedite fee of 1,400 Japanese yen).
      2. Personal Appearance
      3. Duly-accomplished passport application form (Minor form) or Print-Out of confirmed online passport application form (for GOAS)
      4. Original passport and one (1) copy of passport data page (ex. Japanese Passport)
      5. Processing Fee of 8,400 Japanese Yen (cash only)
      6. Original & Copy of Embassy/Consulate Report of Birth, if born in Japan, and/or original  PSA birth certificate, if not born in Japan. If no Report of Birth has been issued, applicants are requested to set an appointment with the Civil Registry Section through email at This email address is being protected from spambots. You need JavaScript enabled to view it. prior to setting an appointment for passport application. Applicants may check the documentary requirements for Report of Birth through this link.
      7. Copy of parent's PSA marriage certificate to establish parental authority.
      8. Copy of passport of the accompanying mother. If accompanied by the father, copy of parent's PSA marriage certificate to establish parental authority.
      9. Self-addressed envelope (nagagata 3) with 660 yen stamps (yubin kite). On said envelope, the name of the applicant MUST BE WRITTEN IN ROMAJI, and do not forget to write your contact number.

Note: Passport processing time is normally two months. Additional requirements maybe requested by the Embassy in compliance with relevant/prevailing rules and regulations as prescribed by the competent authority.


C. FOR Renewal of Green (MRRP)/Maroon (MRP) passports

      1. Schedule passport appointment via the Book Appointment Link 
      2. Personal Appearance
      3. Print-Out of online passport application form.
      4. Original passport and one (1) copy of passport data page
      5. Processing Fee of 8,400 Japanese Yen (cash only)
      6. Original copy of PSA birth certificate
      7. For married women, original copy of PSA marriage certificate
      8. Original and copy of a Philippine-issued government ID with a picture, such as driver's license, voter's ID, Professional Regulatory Commission (PRC) ID, NBI clearance, etc.
      9. Self-addressed envelope (nagagata 3) with 660 yen stamps (yubin kite). On said envelope, the name of the applicant MUST BE WRITTEN IN ROMAJI, and do not forget to write your contact number.

D. FOR Lost Passports:
      1. Schedule appointment by telephone, please call 052 211 8811 (or send an email to This email address is being protected from spambots. You need JavaScript enabled to view it.) and ask to speak with the Lost Passports Desk of the Passports Section.
      2. Personal Appearance;
      3. Duly accomplished passport application form (Adult or Minor form);
      4. Affidavit of Loss (¥3,500);
      5. Report of passport loss from Japanese police authorities (Translation of document: ¥3,500);
      6. Photocopy of data page of lost passport, if available;
      7. Present original and one (1) copy of birth certificate from the Philippine Statistics Authority;
      8. For married women, original and photocopy of marriage certificate from Philippine Statistics Authority (PSA);
      9. Original and copy of a Philippine-issued government ID with a picture, such as driver's license, voter's ID, Professional Regulatory Commission (PRC) ID, NBI clearance, etc.;
      10. Processing Fee of 21,000 japanese yen (cash only)
      11. Self-addressed envelope (nagagata 3) with 660 yen stamps (yubin kite). On said envelope, the name of the applicant MUST BE WRITTEN IN ROMAJI, and do not forget to write your contact number.

There will be a 15-day clearing period prior to the processing of application for replacement of lost valid passport.
