Self-addressed envelope (nagagata 3) with 660 yen stamps (Japan Post yubin kite). On said envelope, the name of the applicant MUST BE WRITTEN IN ROMAJI, and do not forget to write your contact number.

If divorced, original PSA marriage certificate with annotation of divorce, certified true copy (CTC) of the Divorce Decree, and DFA-authenticated certified true copy of Philippine judicial recognition of foreign divorce decree with certificate of finality.
