If not born in Japan:

Original PSA Birth Certificate.

Copy of passport of the accompanying mother and father. If accompanied by the father, copy of parent's PSA marriage certificate to establish parental authority.
- Copy of mother's PSA marriage certificate.

Self-addressed envelope (nagagata 3) with 660 yen stamps (yubin kite). On said envelope, the name of the applicant MUST BE WRITTEN IN ROMAJI, and do not forget to write your contact number.

Schedule passport appointment via the Book Appointment Link or apply for passport same time as Report of Birth (additional expedite fee of 1,500 Japanese yen).
Note: Passport processing time is normally two months. Additional requirements maybe requested by the Consulate in compliance with relevant/prevailing rules and regulations as prescribed by the competent authority.
